It's well known 'the sole objective of any investigation of an accident or incident is the prevention of accidents and incidents. It is not the purpose to apportion blame or liability.' (ICAO Manual)
While the initial field phase of an accident investigation can be concluded within weeks or even days, the investigators' final report and recommendations often take years to complete. The final report consists of factual information about the accident, an analysis, conclusions (probable cause) and also includes safety recommendations. (ICAO Annex 13)
To investigate any accident and determine true root cause, a methodology needs to be established. A methodology uses a discipline, an approach, a structure, and a defendable process to attain success. Without a predefined, well understood approach, the principle investigator and the team will face unfocused efforts, countless questions, and face scrutiny claimants for using an undisciplined approach.
At present, two major methodologies are available for mishap analysis. They are uncomplicated, convenient, concise, and easy to use. They are the 5M and SHEL methodologies.
The 5M model stands for Man, Machine, Mission, Medium, and Management. This model, like the SHEL, is a concise and practical tool to analyze incidents and accidents. It can also be adapted for use in investigating and locating root causes in other industries. Besides Wells 2004 documentation, the US Navy's Aviation Safety School's curriculum utilizes the 5M model. The Department of Defense has recognized this methodology as one of the best diagnostic tools in diagnosing root causes in aviation mishaps. It is taught by the US Navy in its Aviation Safety School curriculum at the Naval Postgraduate School in Pensacola, Florida.
The history of the 5M's roots dates back to the 1940's. T.P. Wright of Cornell University introduced the Man, Machine, Medium model to aviation safety (as cited in Wells, 2004). These three M's are the focus of the original incident investigation model for over twenty years. In 1965, the Management factor of the 5M was introduced by University of Southern California (USC) (http://viterbi.usc.edu/aviation) when the college initiated its aviation safety academic program in their College of Engineering. The final M, Mission, was added to the model in 1976 by E.A. Jerome, a former staff member of the Flight Safety Foundation (as cited in Wells, 2004).
The 5M diagram, replicated from Well's book, demonstrates the interdependencies of the 5M's. Management is the overarching influencer in the model located at the top, encompassing the other four major characters.
Captain Frank H. Hawkins (1987) documents a methodology providing investigators yet another methodology to assist in determining root cause(s), and documenting the findings. The model is called the SHEL concept. Each letter stands for a specific area of study or investigation when dealing with root course determination. S stands for Software, H stands for Hardware, E stands for Environment, and L stands for Liveware. Each part of the model interacts with the Liveware component. Liveware, similar to the Management element in the 5M model, is the center of the model and touches all aspects of the remaining components. All the parts must interact, adapt, and correlate with the center component, Liveware.
As previously stated, L or Liveware, is at the center of the model. The Liveware component refers to the human element within the model. Hawkins refers to it as the hub of the SHEL model. The diagram shows "L" as the focal point in the diagram. Most want to focus on the human element in most mishaps because it's where the investigation usually starts, is usually the root cause, is where fault can be found, the most intriguing part of the mishap, and sells the most newspapers and media formats.
Similar to the Man element in the 5M model, Liveware is the root to all mishaps and is the focus of the most studies within the Human Factors arena. Pictured is the model of the interactions of Liveware with its partners contained by the model.
